  • What is a physician order and why is it required? A physician order (requisition form) is an authorization for you to have lab testing performed. It’s similar to a doctor providing a prescription for medication.

What constitutes a physician order?

  • The physician or allowed practitioner must be enrolled in Medicare;
  • The ordering National Provider Identifier (NPI) must be for an individual physician (not an organizational NPI); and
  • The physician or allowed practitioner must be of a specialty type that is eligible to order and refer.

How to write physician orders?
