  • What was the report on the Challenger disaster?

The commission found that the immediate cause of the Challenger accident was a failure in the O-rings sealing the aft field joint on the right solid rocket booster, causing pressurized hot gases and eventually flame to "blow by" the O-ring and contact the adjacent external tank, causing structural failure.

  • Were any bodies recovered from the Challenger accident?

A congressional investigation of the explosion - which was witnessed by millions watching live television broadcasts of the Challenger launch - concluded that the catastrophic failure was “not survivable.” Recovered remains of the seven Challenger crew members are buried in Arlington national cemetery (here).

  • Who was responsible for the Challenger accident?

Boisjoly correctly predicted, based on earlier flight data, that the O-rings on the rocket boosters would fail if the shuttle launched in cold weather.
Morton Thiokol's managers decided to launch the shuttle despite his warnings, leading to the catastrophic failure.

  • Who was responsible for the Challenger accident?

The autopsy reports have never been released to the public.
There is only one definitive finding ever released, which was that all the Columbia astronauts had microscopic evidence of ebullism (31).
