  • Can you access MyChart internationally?

    Yale Health members can fully access MyChart from the following countries: United States, Puerto Rico, Mexico, United Kingdom, and Canada.
    Mychart is not functional from many international locations; see below for a full list of these locations..

  • If you forget your username, you will be asked to verify your first name, last name, date of birth and ZIP code.
    Once you click the submit button, an email from mychart@ohsu.edu will be sent to your email containing your MyChart username.
    The email subject will say “MyChart Verification code."
  • To have your account re-activated, send an email request to MyChart@CentraCare.com or call our MyChart Patient Support Line at 320-240-7897.
