Who is eligible for a referral to the CMHT?
Referral.
Referral to the CMHT should be considered for patients with suspected or established moderate to severe mental illness / mental disorder who for reasons of complexity, severity or lack of treatment response require specialist input (see appendix 1)..

The Primary Mental Health Support Services (PMHSS) is a dedicated service for adults aged 18 and over who are dealing with common mental health challenges such as depression and anxiety.
We welcome referrals from General Practitioners (GPs) and other mental health teams and professionals.

Thanet Community Mental Health team provides support and treatment in the community to adults between the ages of 18-65 who are experiencing a mental illness. The team provides an assessment of an individual's current needs and provides treatment for service users with moderate to severe mental health needs.

The team includes psychiatrists, community mental health nurses, occupational therapists, psychological services and support staff and also works in partnership
