amide ester local anesthetic allergy
When should an amide be considered for a dose test?
In patients who have had reactions to an ester-type local anesthetic, an amide should be considered for provocative (graded) dose testing. If the patient had a reaction to an amide, another amide might be considered because cross-reactivity among amides has not been documented.
Do amide-type Las cause allergies?
Conversely, allergies to amide-type LAs have been rarely reported. Lidocaine is the most common anesthetic agent used in numerous surgical procedures for local and regional anesthesia and is the gold standard compared to other LA agents. Herein, we present cases of two patients with allergy to lidocaine and describe their management.
Can methylparaben and metabisulfite cause allergic reactions to local anesthetics?
A true immunologic reaction to a local anesthetic is rare. Intradermal skin testing of local anesthetic compounds, methylparaben, and metabisulfite should be performed in patients when a thorough history does not rule out a possible allergic reaction to local anesthetics and future local anesthesia is necessary.
