The “Analgesia Nociception Index” (ANI; MetroDoloris Medical Systems, Lille, France) is a proposed noninvasive guide to analgesia derived from an electrocardiogram trace. ANI is scaled from 0 to 100 ; with previous studies suggesting that values ≥50 can indicate adequate analgesia.
What is analgesia nociception index (Ani)?
The Analgesia Nociception Index (ANI) (based on heart rate variability [HRV]) and the Nociception Level Index (NOL) (based on HRV, photoplethysmography, skin conductance, and temperature) both include HRV and provide continuous index monitoring for nociception assessment.
Is Ani a useful nociception/pain monitor?
In this article, we reviewed key papers on ANI that have been published in the last decade, introduced HFVI, and discussed the outlooks of ANI/HFVI. At present, ANI has a certain usefulness as a nociception/pain monitor during or immediately after general anesthesia in patients undergoing surgery, and in severely ill patients in the ICU.

Changes in the Analgesia Nociception Index reflect nociceptive stimuli during surgery and response to administration of analgesia such as opioids. Analgesia Nociception Index is comparable to other objective pain assessment tools and better than conventional methods for assessment of pain during surgery.

The available literature show that the ANI and NOL perform similarly for nociception assessment in anesthetized patients undergoing surgery. Future research in other contexts of care and populations of noncommunicative patients should be conducted.
