PDF UNC Childrens Febrile Neonate/Infant Clinical Pathways 0-21 days

14 jan 2022 · This clinical pathway for the evaluation and management of well-appearing febrile infants at UNC Hospitals was adapted from the 2021 AAP

  • The Rochester criteria were developed to identify febrile infants aged 60 days or younger at low-risk of bacterial infection and do not include cerebrospinal fluid (CSF) testing.
Prior studies have not specifically assessed criteria performance for bacteremia and bacterial meningitis (invasive bacterial infection).

  • What is the pathophysiology of neonatal fever?

Pathophysiology of Fever in Infants and Children
Fever occurs in response to the release of endogenous pyrogenic mediators called cytokines (in particular interleukin-1 [IL-1]).
Cytokines stimulate the production of prostaglandins by the hypothalamus; prostaglandins readjust and elevate the temperature set point.

  • What is the protocol for a 1 month old fever?

Also, contact your child's provider or go to the emergency room if your child: Is younger than age 3 months and has a rectal temperature of 100.4°F (38°C) or higher.
Is 3 to 12 months old and has a fever of 102.2°F (39°C) or higher.
Is under age 2 and has a fever that lasts longer than 48 hours.

  • What age do babies need spinal tap for fever?

Because these patients will undergo empiric treatment with antibiotics, some experts advocate for a lumbar puncture in all febrile infants 29 to 90 days of age with a focal infection to minimize the risk of missing bacterial meningitis, especially patients with focal infections likely to be caused by group B
