Do I need an ABN for a laboratory test?
When Medicare considers an item or service experimental (e.g., a “Research Use Only” or “Investigational Use Only” laboratory test), payment for the experimental item or service is denied under §1862(a)(1) of the Act as not reasonable and necessary. In circumstances such as this, the beneficiary must be given an ABN.
Who must complete the ABN?
“Notifiers” include: All of the aforementioned healthcare providers and suppliers must complete the ABN as described below in order to transfer potential financial liability to the beneficiary, and deliver the notice prior to providing the items or services that are the subject of the notice.
Do SNFS need an ABN?

Can a provider use a Medicare ABN?
With the exception of DME POS suppliers (see Section 50.10), providers and suppliers who are not enrolled in Medicare cannot issue the ABN to beneficiaries. Provider use of the ABN has expanded to include home health agency (HHA) issuance for Part A and Part B items and services.
