What is an advance directive in NY?
Advance Directives are a legal documents by which you may make provisions for future health care decisions in the event that you are unable to make such decisions for yourself.
In New York State there are three types: Health Care Proxy form, Living Will, and Do Not Resuscitate Order (DNR).All competent adults, 18 years of age or older, can appoint a health care agent by signing a form called a Health Care Proxy.
You don't need a lawyer or a notary, just two adult witnesses.
