The diabetic foot and its diagnosis

Poor metabolism over the years in people with diabetes can lead to different secondary and accompanying disorders, for example, diabetic foot syndrome. This disease is marked by nerve damage and circulatory distur- bances in the legs and feet. These symptoms develop in about 15% of all those affected by diabetes. Typical for the diabetic foot is very poor wound healing and the high tendency to recurrence. Healing is only possible with patienceandexperttreatment.Forthisreason,prevention is of immense significance. It is considerably less radical and more economical to avoid an ulcer in the first place, than to have to heal one later on or even to have to ampu- tate. In order to prevent foot wounds, the right shoes are of great importance. Most foot lesions are caused from

wearingill-fittingshoes.In order to thwart the risk of amputation, regular visits toa foot care centre are indispensable, in addition to thedaily control of the feet and shoes if necessary, also with

the help of a family member. Besides visual checks, various diagnostic aids have proved to be of value: The vibration sensation of peripheral nerves can be checked with the help of a tuning fork, as a criterion for depth sensitivity and to quantify nerve loss on the basis of scaling. The surface sensitivity can be tested with the help of a monofilament. If no pressure is felt, the touch sensitivity is already strongly limitedandthenaturalprotectivefunctionalreadyaffected. The Tip Therm checks the temperature sensitivity of the feet. The loss of the sense of temperature sensitivity is a decisive early symptom for incipient polyneuropathy. The timely detection of this symptom means an important gain in time in prevention and treatment. Pressure peaks are often the trigger for the occurrence of plantar food wounds. With an electronic foot pressure measurement, the quality of individual care can be optimised in many different ways. This system is particularly advantageous, amongst other things, for direct construction of insoles in the PC and simple docu-

mentationforevaluatingtheprogressoftreatment.Systemsofvarioussuppliershaveprovedtobeadvantage-ous for dynamic measurements, which show the pressuredistribution in the shoe between the sole of the foot andthe insole. They enable exact recording of the load underthe foot and help to recognise changes or defectivepositioning of the feet. A pressure measurement in the

shoe is required by the compulsory health insurance companies for the production of diabetes-adapted foot supports, as verification of pressure reduction by the medicalappliance.

These shoes are made with the same high quality materials as LucRo classic shoes. Due to the way they are constructed and equipped, they offer ideal protection following the first signs of foot wounds in persons with PNP. In this way, an economical form of preventative care can be provided. The main characteristic of the LucRo kinetic series is a changed fit compared to LucRo classic shoes. The insole shape is somewhat than that in the LucRo classic, especially in the hind-foot. Also, the heel arch was adapted in geometry to fit a narrower hind-foot. The LucRo kinetic also pays particular attention to highest quality and a pleasing design. Tests carried out by renowned institutes on the biocompatibility of the shaft materials, sole abrasion resistance and additional fatigue bendingtrialswereabletoproduceconvincinglypositiveresults.
