When should I take Step 3?

Generally, you can take Step 3 at any point during your intern year, during any rotation. It is easiest to take the exam during an outpatient block or consult service because most often you will not need to find coverage during these times. The exam is generally scheduled 3-6 months in advance of the exam date. It is your responsibility to let the OHSU schedulers know your exam date at least 6 weeks in advance. If you plan to take the exam during VA or OHSU wards, MICU, or jeopardy blocks you will have to find coverage while you take your exam. While taking the exam during clinic week or a consult service generally protects you from finding work coverage, you will still need to find coverage if you are scheduled for jeopardy on the dates you have selected to take the exam. It is often easier to find jeopardy coverage than it is to find work coverage. When should I register and schedule my Step 3 Exam? Most interns will strive to complete their Step 3 exam by the end of their first year of residency.

In fact, GME requires that all residents register for Step 3 by the 12th month of their training and

must take and pass the exam within the first 18 months of training. Once your registration is complete (see below), you will be able to schedule exam dates within your selected eligibility period. What about registering and scheduling for COMLEX Level 3? If you are registering to take the COMLEX Level 3, follow these steps:

1. Purchase the exam on NBOME.org. The exam you purchase will have an active range.

The data range you purchase will only allow you to schedule examination dates within the range. E.g. if you choose 7/1/2017 - 6/30/2018, you may not choose any date after

6/30/2018 or before 7/1/2017.

2. Go to the Prometric website and search for the scheduling section.

3. Click on 'Scheduling and Registration.'

4. Follow the steps listed on Prometric to determine which eligible dates exist for the exam.

5. Compare the available exam dates (you will need two days) to your schedule on Amion

(refer to below for help deciding which is the best date to take the exam based on your schedule)

6. Choose a date and exam location. There are two exam locations in Portland.

7. Email the scheduling department with subject headline: Step 3 Schedule Request and

let the scheduling team know which date you are requesting to take Step 3

8. Make any necessary scheduling swaps or coverage exchanges required

How much does Step 3 cost?

The application fee in 2018 was $850 for USMLE and $875 for COMLEX. Unfortunately, neither GME nor the department of Internal Medicine offer financial assistance for these fees.

When should I prepare for Step 3?

Determining when to prepare for Step 3 depends on your duration of preparation and more importantly your residency schedule. Regarding length of preparation, most interns will spend 1 month (or longer, depending on level of readiness) to study after work and on weekends. With that in mind, it is best to find a block in your schedule of consecutive outpatient time in which you will have weekends off to commit to studying. An alternative strategy would be to schedule the exam far in advance and do small blocks of questions when able, regardless of your current rotation, and then ramp up your studying 1-2 weeks prior to exam. If you choose this strategy know that it is challenging to dedicate study time during inpatient wards and ICU rotations.

How should I prepare for Step 3?

Like Step 1 and Step 2, we believe that the single most effective method of preparation entails doing lots of practice questions. For USMLE, the gold standard is UWorld and for COMLEX, COMBANK seems to be the QBank of choice. Unlike Step 1 and Step 2, Step 3 has an additional section dedicated to clinical cases. The clinical cases on the USMLE are not the most intuitive and for the COMLEX, 2018 will be the first year they incorporate them. Needless to say, these questions take some getting used to and it is best to purchase the additional clinical case simulations with each test bank and do them in advance of your scheduled exam date. Step 3 exam review books are available at the library and if you are so inclined you could skim these books.

How important is getting a good score?

Most importantly you want to pass Step 3. You should strive to do your best on the exam, but realize that you will not have as much time to devote to studying as you did for Step 1 and Step

2. If you are applying for fellowship the score may matter more for you in the future, but many

fellowships prioritize other aspects of your residency portfolio and equally consider your performance on Step 1 and Step 2. Do your best and leave the rest.

What should I do when I have my score report?

Once you have officially passed, celebrate! Then, once you have received your score report, forward a copy to GME. This will be included in your GME portfolio. Additionally, once you have passed Step 3, you will be eligible for an UNLIMITED LICENSE should you wish to obtain one.
