Brief Program Description

The Australian Nurse

-Family Partnership Program (ANFPP) is a nurse-led home visiting program based on the US Nurse Family Partnership. The program empowers and informs first-time Indigenous mums or mums whose partners Aboriginal or Torres Strait Islander and support them to reach their personal goals and grow strong, healthy families. A t the heart of ANFPP is an acceptance of client autonomy; the client is the expert in

her own life and she can identify solutions that work for her. The 5 client centred principles play a pivotal role by

focusing on her strengths, focusing on solutions, understanding only a small change is necessary, realising that the client is the expert and empowering the client to follow her heart"s desire. The underlying objective of the ANFPP is to improve the health, wellbeing and self-sufficiency of young parents and their children using that client-centred, strengths based, solutions focused approach. ANFPP Nurse Home Visitors, Nurse Supervisors and Family Partnership Workers apply the theoretical framework that underpins the program (self-efficacy, human ecology, and attachment

theories) to guide their clinical work and achievement. The first goal is to improve outcomes in pregnancy by working alongside and supporting women to

address their health needs and engage in effective preventative health practices. The second goal of the program is targeted at improving child health and development by

working with parents to support them to be the best parents possible. The third and final goal of the program is to improve

parental life course by working with and supporting parents to develop a vision for their own future, family planning, continued education and employment. The program also strives to include fathers, partners and other significant family members where possible.

Current Service Delivery

Each site has a home visiting team which consists of a Nurse Home Visitor and a Family Partnership

Worker (which is an identified Aboriginal and/or Torres Strait Islander position). The home visits are

conducted by the Nurse Home Visitor and Family Partnership Worker as a team who visit the mother in her home or in a location chosen by her. Here, they deliver a prescribed education curriculum while aiming to match the goals of the client to those of the program. The education component of the home visit is split into pregnancy, infancy and toddlerhood phases and each phase covers five domains that influence the health and wellbeing of the mother.

Using education resources such as

Yarning tools, the team delve deeper to uncover specific topics the mother is seeking further

support in. The yarning tools encourage the mother to reflect on how she is going, practice self-care

and introduce habits and lifestyle changes to accommodate improvements in her life.

Abt/NPC:

The National Program Centre, otherwise known as the NPC, supports the partner organisations to deliver the program. Following recruitment, the home visiting team are trained in Brisbane by our diverse NPC Workforce Development and Education team of experienced Nurses, Midwives and Family Partnership Worker Educators. The purpose of training is to support staff with education and skills to enable home visiting and program delivery.

We also encourage staff at various sites to learn

from each other. Monthly role specific community of practice meetings are facilitated by the N

PC via

teleconferences and online discussion forums.

The NPC also provide networking and professional

development opportunities for the staff at sites through workshops and the annual National ANFPP Conference which hosts over 100 staff from all 13 sites.quotesdbs_dbs29.pdfusesText_35
