KEYWORDS: Gas exchange, PaO2, PAO2, arterial blood gasThe alveolar gas equation predicts alveolar oxygen pressure according to partial pressure of oxygen in inspired gas and
partial pressure of arterial carbon dioxide.Based on alveolar oxygen pressure, the alveolar-arterial oxygen gradient, an index used to differentiate respiratory failure
etiology and evaluate lung injury severity, is calculated. Although alveolar-arterial oxygen gradient is affected by some
physiologic and pathologic factor, it is the first and simplest step in assess lung gas exchange. The evaluation of gas exchange effectiveness plays an important role in assessing severity of lung diseases. The main
parameters estimating blood oxygenation are the difference or the ratio between arterial pressure of oxygen (PaO2) and the
partial pressure of oxygen in alveoli (PAO2).In clinical practice, PaO2 is easily measured by analysis of arterial blood gas (ABG), while we can only estimate PAO2 by
direct measurement of partial pressure of oxygen in expired air or by calculation of PAO2 from alveolar gas equation.
