For a course that requires a prerequisite, a student who has not met the prerequisite can be added to the course ONLY with instructor permission. Please note that this form only applies to students who have not met the prerequisite in some way. If you have met the prerequisite (e.g., through AP/IB scores), but it is not yet showing up on your transcript, you can bring documentation of your scores to psychology advising and do not need instructor permission. To obtain instructor permission, please complete this form as follows: (1) Fill out the top half of the form completely. (2) Take this form to instructor, and have him/her complete the bottom half of the form. Alternatively, you can attach an email from the professor that clearly indicates his/her permission to waive this course prerequisite for you specifically. (3) Bring this form to psychology advising (PSY 135) in person.
