CFA LITTER REGISTRATION APPLICATION INSTRUCTIONS

General

Litter registration is required if ANY kitten is to be individually reg- istered either now or in the future. This rule ALWAYS applies, even if there is only one living kitten and/or if one or more kittens are being individually registered. Litter registration is encouraged at the time the litter is born even if no kittens are being individually registered at that time. Individual kit- ten registrations can be processed more quickly if a litter has been previously registered. The official CFALitter Registration Application or facsimile must be used. All signatures must be originals. Additional application forms are available from the Central Office at no charge and on CFA's website, www.cfa.org. PRINT or TYPE except where signatures are required. Signatures, where required, attest to the accuracy of the informa- tion contained on this application. Applications will be returned unprocessed for incorrect CFA registration numbers and/or fees. Errors or delays may occur because of illegible writing - please

TYPE or PRINT all information except signatures.

The completed litter registration certificate and individual regis- tration applications for each unregistered kitten in the litter will be mailed to the breeder. However, if the litter owner section is complet- ed, all forms will be sent to the Litter Owner.

Section A • Birth Information - R

EQUIRED

This section must be completed by the breeder of the litter. The breeder is the person(s) who is registered with CFA as the owner/lessee of the dam on the date of mating. After registration, if the number of male and female kittens changes, but the total num- ber of kittens is the same, it is not necessary to contact our office.

Section B • Sire Information - R

EQUIRED

This section is to be completed by the owner/lessee of the sire at the time of mating. If the sire was on lease at the time of mating, see

Leases for further instructions.

Section C • Dam Information - R

EQUIRED

This section is to be completed by the owner/lessee of the dam at

the time of mating. If the dam was on lease at the time of mating, see Leases for further instructions. The cattery number/name entered

must be the cattery name of the breeder and will appear as the prefix on all kitten names.

Leases

If the sire and/or dam was leased to you on the date of mating, you (the lessee) are the breeder of the litter and should complete the first part of Section B and/or Section C. A lease must be recorded with CFA if the owner(s) of record leases a sire or dam to an individual(s) with the intent the registered owner's name(s) be excluded from all transactions during the specified lease period. The lease recording fee and a copy of the lease agreement, to include the name and reg- istration number of the cat, name of lessee(s), lease period (from-to) and signatures of ALL registered owner(s) must be sent to CFA prior to or in conjunction with the processing of the first transaction. If any or all of the registered owners are to be included as lessees on a litter registration, the optional lease section on the litter applica- tion form must be completed by the registered owner of the sire/damquotesdbs_dbs7.pdfusesText_5
