Community health pay online

Someone from the provider’s office will contact you with an appointment

Use a credit or debit card to pay your bill through your Patient Portal account and your payment will be applied directly to the balance on your Community Health account

View what is currently in your history document and make any necessary changes

To send a payment by mail, tear off the bottom of the PCHC invoice at the perforation and fill out your information

Then send this form with your payment in the envelope provided
