isolates[10].Moreover, theexact timeofantibiotic switchingwas notrecordedin allstudies anddifferentdefinitions ofswitching wereused[12,13]. Inthis studyonlythe identificationtimewas shortenedwhilethe durationof susceptibilitytestingwas left unchanged.Therefore,any changein theproportionof patients onadequatetreatment canbe attributedtothe useofdirectMALDI-TOFMSanalysis.
Contributionofmicrobiology resultsto antimicrobialmanage- mentdependson localantimicrobial policiesandbacterial ecology.Thisstudy wasperformed inasetting whereantibiotic resistanceisinfrequent, whichallows speciesidentificationto bea morepowerfultool thanin settingswithmore complexresistance patterns.Inadequateempirical therapyoccurs infrequentlyinour populationbecauseof lowresistance levels.Forinstance, there werenobacteremia episodeswith MRSAandvancomycin resistantenterococciand empiricaltherapy wasadequatein the majorityofpatients alreadyreceiving antimicrobialtreatment. Inthisstudy antimicrobialtherapy wasjudgedinappropriate whenisolatedpathogens wereresistant toprescribedagents or whenantibiotictherapy wasnot accordingtothe hospital guidelines.Consequently,a fewantimicrobial regimensjudged inappropriatemay,although suboptimal,be effectiveagainstthe causativepathogens.In thisstudy, directMALDI-TOFMS yieldedcorrectidentification ina lowerproportionof episodes comparedtothe resultsof previousstudies.This mightbe explainedbya relativelylarge proportionofGram positiveand polymicrobialinfections inthisstudy, whileinother studies polymicrobialsampleshave beenexcluded[6]. Itisknown that Grampositiveand polymicrobialinfections areoftennot accuratelyidentifiedby directMALDI-TOF MS[14].However, adaptationsinsample processingmay increaseratesof correctly identifiedmicroorganisms[7,15-17] andthereby increaseclinical impactofdirect MALDI-TOFMSeven further. Limitationsofthis studyincludethe factthatthe studysizewas toosmallto evaluatedifferences inmorbidityor mortalitybetween theinterventiongroup andthe standardcaregroup. Inaddition, MALDI-TOFMSonly providesidentification withoutinforma- tiononantimicrobial susceptibility.Thisinformation canguide antibioticmanagementbetter ina settingwithlow levelsof antibioticresistancecompared toareas withhigherlevels of resistance.Thismay decreasegeneralizability ofourdata to countrieswithhigher levelsof antibioticresistance. Inconclusion,the useofdirect MALDI-TOFMSon positive bloodculturesresulted inafaster identificationofmicroorganisms causingbloodstreaminfection.
